IMPORTANT MEDICAL INFORMATION
Medical services in Greece may not be covered by your health plan in the country you reside.  The Panmessinian Federation is not responsible for any health costs you may incur while attending the Summer School in Kalamata.  It is the responsibility of applicants and a requirement for your registration to obtain additional travel health insurance and to provide the trip supervisor with a confirmation number before the trip (required). 

1)    Have received and understood all information related to this trip.

2)    Agree to fulfill all expectations and follow the rules outlined by the University and Panmessinian Federation program for the sound functioning of the school and my safety.

3)    Authorize representatives of the University of Peloponnese and/or Panmessinian Federation to obtain all medical or other aid for me or my son/daughter (if applicant is under the age of 18) which may be required in case of an emergency.

WAIVER RELEASE FORM

RELEASE INDEMNIFICATION AND HOLD HARMLESS AGREEMENT 

WHEREAS, the undersigned Applicant, being of lawful age, wishes to participate in the Summer School Program in Kalamata, Greece; (hereinafter referred to as the “Program”) offered by the Pan-Messenian Federation USA & CANADA, Inc., in cooperation with the University of the Peloponnese in Kalamata, in Messenia, Greece, (hereinafter referred to collectively the “Released Parties”). 

NOW THEREFORE; in consideration of ten ($10.00) and other good and valuable consideration, paid in hand and mutually acknowledged by the respective parties; and in order to induce the Federation and the University of the Peloponnese in Kalamata, in Messenia, Greece to allow the participation of the Applicant in the Summer School Program in Kalamata, the parties hereby mutually acknowledge and mutually agree to the following: 

Applicant, being of lawful age, his/her family, legal representatives, heirs, executors, administrators, successors, and assigns and agents, (collectively referred to as the “Releasing Parties”) agree to Release, and Forever Discharge, Indemnify and Hold Harmless the Pan-Messenian Federation USA & CANADA, Inc., the University of the Peloponnese in Kalamata, Messenia, Greece; as well as their respective officers, directors, attorneys, representatives, employees, associates, partners, agents, servants, insurers, successors, heirs, executors, administrators and all other persons, corporate associates, or partnerships or any other entity connected therewith (collectively, the “Released Parties”) from any and all claims, known or unknown, tangible or intangible, actions, causes of action, demands, obligations, liens, rights, damages, costs, loss of service, expenses and or compensation of any nature whatsoever which the Releasing Parties now have or which may ever have or which may accrue to the Releasing Parties on account of Applicant’s participation in the Summer School Program, or in any way growing out of, and any and all known or unknown, foreseen and unforeseen injuries and or damages and the consequences thereof, including death, or any injuries resulting from or to result from participation in the Summer School Program in Kalamata, Messenia, Greece.  

This Agreement contains the entire written agreement between the Releasing Parties and the Released Parties with regard to the matter set forth in it and shall be binding upon and inure to the benefit of the parties hereto, jointly and severally, and the executors, administrators, personal representatives, heirs, successors, and assigns of each.  

 

The Releasing Parties and the Released Parties mutually agree that this Agreement as entered into shall be construed and interpreted in accordance with the laws of the State of Illinois, United States of America, and any matters, controversies, claims, arising from this Agreement shall be heard and adjudicated in the State of Illinois, County of Cook, Chicago, Illinois, First District, United States of America, which shall be the only proper forum for all matters arising from this Agreement. 

In entering this Agreement, the Releasing Parties represent that they have sought and relied upon the advice of their attorney who is an attorney of their choice, and that the terms of this agreement have been completely read and explained to them by their attorney and that those terms are fully understood and voluntarily accepted by them.

PANMESSINIAN FEDERATION OF USA AND CANADA
GREEK SUMMER SCHOOL 2024 

A brief Letter of Reference (from a non-family member, preferably a member of an educational institution) must be submitted outlining the character and personality of the applicant, his/her ability to follow rules and instructions, the ability to understand and communicate in the Greek language and how the student will benefit from the Summer School Program.

Code of Conduct - Student Responsibilities

1. Students are expected to attend all classes from the beginning to the end of the program and be on time.  They must remain in the class until the lesson is complete unless there is an emergency.

2. Students are to conduct themselves in a manner that will ensure safety to themselves and others.

3. Students are expected to listen and give their undivided attention to their teachers.

4. No talking in class with others unless instructed by teacher.

5. Students will take care of the classroom and any materials provided by the University.

6. No cellular phone usage in the classrooms.

7. No sleeping, eating or drinking in class (except water).

8. When visiting Archeological Sites, Museums or any other places deemed necessary by the program, please stay with your group.  It is important to listen to the guides and teachers. You are “students” not tourists at this time.

9. The Panmessinian Federation of U.S.A. & Canada is not responsible for students after school hours unless they participate in a school activity.  A separate legal form must be signed by parents, guardians or students of age.

10. Students must inform us if they have any medical problems.  We need to know how to help you in case of an emergency. Complete and sign the required medical form.

11. Only participants of the program are allowed in residence.

12. Curfew: Sunday to Friday 1:30 a.m.; Saturday: 3:00 a.m. STUDENTS MUST STRICTLY COMPLY WITH THE CURFEW

13. No alcohol drinking in residence. (No heavy drinking outside school hours).

14. There are no exceptions for time off during the program. Absences are not allowed unless for medical or family emergencies.

ALL STUDENTS WILL BE RESPONSIBLE FOR THEMSELVES DURING NON-SCHOOL HOURS, but students are still part of the program and therefore they must follow the school rules and guidelines set by the Panmessinian Federation of USA and Canada at all times. 

Students must be courteous, polite, sensible and respectful to their peers, University instructors, Greek teachers, officials and all Panmessinian Federation Representatives.

Failure to comply with any of the above student responsibilities may lead to the immediate dismissal from the program at the student’s expense.
